I use Credential Direct, which is a broker associated with the Credit Union system. They have Level II and as far as I know do not have an extra charge.

Alright, I made a few calls . Here's what I've found so far.
BMO - Canadian and US exchanges free with accounts over $250k (streaming data at over $500k)
Scotia - Canadian and US exchanges free with accounts over $250k
Credential - Canadian exchanges free with all accounts (no US exchanges, regardless of fees / account size). No minimum account size ($15k minimum to avoid the quarterly $25 administration fee)
Qtrade - Canadian exchanges free with all accounts (no US exchanges, regardless of fees / account size). No minimum account size ($25k minimum to avoid the quarterly $25 administration fee)
For US exchanges, I came across this link in another thread: http://www.bats.com/us/equities/market_statistics/book/AAPL/
Perhaps someone with Level 2 data from their brokerage can check how good / accurate it is. Note that there are four different Book Viewers (links at the top), which show different data.
